Problem:
Denials occurred when both chiropractic manipulation codes (98941 and 98943) appeared in the same paragraph of the clinical records. If the claims analyst didn’t read closely, the second code was often missed and denied.

Solution:
When uploading medical records, we now include a note referencing the exact page, paragraph, and sentence where both codes are documented. This ensures clarity and reduces confusion for payers.

2. Labeling X-Ray Views Clearly and Separately
